How do I schedule an appointment?

There are two ways to schedule an appointment online, with scheduling tickets, preselected appointment types chosen for you by your provider, or directly with your Primary Care Provider. Both scheduling methods are available on the Schedule an Appointment page, but you will only see scheduling tickets if you have an active ticket.

What kind of appointments can I schedule?

Can I view a family member's health record in MyChart?

A process called "proxy access" allows you to view the MyChart records of a family member or someone under your care. First, the patient or the patient's legal representative must complete the proper proxy authorization form at his/her provider's office to give another individual permission to access their medical information through MyChart. Proxy access is granted for parents with children under the age of 13 or for an adult caregiver of a family member. Complete a Proxy Consent Form at your physician's office to request access to this convenient service.

Yes, you can. MyChart proxy access allows you to access the medical records of your family members and others you care for, with their permission. You must be at least 18 years old to have proxy access to another person’s medical record.

You might also want to grant a family member or friend access to your medical records when you need assistance managing your appointments and other medical needs. To give someone access to your medical record, grant access via a proxy invite through the Friends and Family Access page in MyChart.

Can I ask questions regarding a family member from my MyChart account?

MyChart offers direct access to your personal health record and communicating about another individual's information would cause that information to be placed in your own health record. The information would not appear in the correct health record and could potentially jeopardize medical care. For this reason, we discourage this avenue of communication.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each individual must establish their own MyChart account. Also, as stated above, communications via MyChart become part of your medical record so you each need to have your own account.

If you're having trouble logging in, click the “Forgot Username?” or “Forgot Password?” link below the login fields for assistance. You will go through two-step verification to verify your identity so you can recover your username or password. You can also contact our MyChart Patient Support Line at 833-254-3600 to request assistance.
